About the Job

We are seeking a skilled Social Worker I to join our team at Houston Methodist. As a key member of our healthcare team, you will provide comprehensive clinical social work and discharge planning services to patients and their families.

Key Responsibilities
  • Collaborate with physicians and the interprofessional healthcare team to provide patient-centered care and achieve desired treatment outcomes.
  • Conduct thorough assessments of patients' social and emotional needs, identifying areas for intervention and developing effective treatment plans.
  • Work closely with case managers and other healthcare providers to ensure seamless transitions and effective discharge planning.
  • Provide education and support to patients and families, empowering them to make informed decisions about their care.
  • Stay up-to-date with community resources and healthcare financial and payer issues, ensuring that patients receive the best possible care.
Requirements
  • Master's degree in Social Work (MSW) from an accredited program.
  • Current licensure as a Licensed Medical Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the State of Texas.
  • 0-1 year of experience in hospital social work, with a hospital internship preferred.
