Job Responsibilities- Assist in the implementation of the CalAIM for Justice Involved Initiative, Whole Person Care Program, and Complete Care Model.
- Collaborate with the healthcare team and other stakeholders to ensure the delivery of timely and appropriate pre-release health care services to eligible patients.
- Address the ongoing pre-release needs of patients and provide education on health and wellness.
- Work closely with the Nurse Consultant III (Supervisor) and Supervising Registered Nurse II (SRN II) to ensure seamless care coordination.
- Participate in training and development opportunities to enhance your skills and knowledge.
- Possession of a valid, active license from the State of California to practice as a Licensed Vocational Nurse.
- Ability to work collaboratively as part of a team.
- Strong communication and interpersonal skills.
