About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our dynamic team at JFS. As a Care Coordinator, you will play a vital role in providing care coordination services to adults enrolled in our Comprehensive Community Services (CCS) program.

Key Responsibilities
  • Meet with individuals in their homes or out in the community to identify recovery goals and develop customized recovery plans.
  • Work with consumers and a dedicated team of providers to decide which programs and supports will assist the individual in reaching their recovery goals.
  • Conduct assessments, monitor, and support CCS consumers with a professional and caring methodology.
  • Collaborate with community professionals to ensure comprehensive care and support for consumers.
Requirements
  • Four-year Bachelor's degree in a Human Services field (e.g. Social Welfare, Psychology, etc.)
  • Minimum of one (1) year of paid formal (non-family setting) experience working with adults providing Case Management or related services
  • Experience in behavioral health, mental health, and/or addiction is preferred
  • Excellent interpersonal, organizational, critical thinking, problem-solving, creative, and independent thinking skills
  • Ability to work independently with minimal supervision
  • Excellent verbal and written communication skills
  • Ability to maintain consumer confidentiality and respect their dignity at all times
