Key Responsibilities- Initial Nursing Evaluation: Conduct thorough assessments of patients to determine eligibility for hospice services within 48 hours of referral.
- Plan of Care Development: Collaborate with patients, families, and healthcare teams to create personalized care plans that address physical, psychosocial, and environmental needs.
- Continuous Care Coordination: Ensure seamless care delivery by coordinating with healthcare providers, patients, and families to address changing needs and concerns.
- Accurate Documentation: Maintain detailed and accurate clinical notes, problem lists, and patient/family responses to hospice care.
- Interdisciplinary Collaboration: Work closely with patients, families, attending physicians, and other healthcare professionals to provide comprehensive care and support.
- Experience: One year of clinical experience as a Registered Nurse.
- Licensure: Current RN licensure in the state of practice and one year of clinical experience.
- Certifications: Current CPR Certification and valid driver's license with vehicle insurance.
