Job Summary

We are seeking a skilled Social Worker, MSW to join our team at Dartmouth Health. As a key member of our healthcare team, you will provide assessment, counseling, and resource assistance to patients and their families.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments of patients and families to identify their needs and develop effective care plans.
  • Provide counseling and support to individuals and families to help them cope with social and emotional challenges.
  • Coordinate community resources to address the needs of patients and families, ensuring they receive the support they require.
  • Assist families in adapting to difficult and complicated situations, providing guidance and support as needed.
  • Provide consultation, assessment, and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
  • Organize support groups to help patients and families connect with others who share similar experiences.
  • Document all clinical data in the medical record, ensuring accurate and timely information is available to the healthcare team.

Qualifications

  • Master's Degree in Social Work (MSW) required.
  • Experience in an acute, home care, or ambulatory setting preferred.
  • Strong organizational skills, verbal and written communication skills, and sound decision-making and judgment.
  • Ability to manage time effectively, negotiate, and resolve conflicts.
  • Comfortable with basic computer skills and ability to learn new software applications.
