About the RoleThe successful candidate will be responsible for evaluating, planning, implementing, and documenting patient treatment plans for home care patients. This role requires excellent communication skills, both written and verbal, as well as strong clinical knowledge and technical skills. As a member of our team, you will work closely with Physical Therapy Assistants and other healthcare professionals to ensure seamless care delivery.
Responsibilities- Evaluate patient status and develop comprehensive treatment plans tailored to individual needs
- Implement and document treatment plans, ensuring compliance with regulatory guidelines and agency standards
- Supervise Physical Therapy Assistants and cosign their documentation as required
To be considered for this role, you must have a Physical Therapy License and the ability to relocate to Nassau County, NY before starting work.
