Home Health Occupational Therapist Specialist
About the Role:BAYADA Home Health Care is seeking an experienced Occupational Therapist for a full-time opportunity performing home health visits in York, Pennsylvania. As a member of our multi-disciplinary healthcare team, you will provide rehabilitative care and skilled nursing to clients, enabling them to receive medical attention required to remain at home.
Responsibilities:- Make home visits to clients in designated geographic territories.
- Perform thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes.
- Evaluate home environment for hazards or barriers to independent living.
- Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients.
- Continually assess and revise occupational therapy care plans, participating with nursing in the multidisciplinary care plan as needed.
- Educate and instruct patients, family members, or other patient representatives on necessary rehabilitative care and activities promoting client health, safety, and independent living.
- Accurately document observations, interventions, and evaluations related to patient care management and services provided using state-of-the-art touch pad tablets.
- A current PA Occupational Therapist license.
- Prior home care experience preferred but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and AOTA, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience using electronic medical records (EMR) preferred.
$60,000 - $90,000 per year, depending on qualifications and experience.
