Home Health Physical Therapist Specialist

JAG Physical Therapy is dedicated to providing personalized rehabilitation services to patients in their home environment. As an At-Home Physical Therapist, you will play a vital role in improving patient function and quality of life. With a focus on evidence-based practice, you will conduct assessments, establish treatment plans, and collaborate with an interdisciplinary team to ensure seamless care.
The ideal candidate will possess strong clinical decision-making skills, excellent communication abilities, and a commitment to delivering exceptional patient care. You will be responsible for collecting and recording patient care data, preparing evaluations, administering tests and measures, and synthesizing results to formulate diagnoses, goals, and plans of care.
Key Responsibilities:
- Develop and implement comprehensive treatment plans tailored to individual patient needs
- Conduct thorough assessments, including initial evaluations, daily notes, reassessments, and progress reports
- Collaborate with the interdisciplinary team to ensure coordinated care and achieve optimal outcomes
- Provide education and training to patients and their support systems on ADLs/IADLs, therapeutic activities, and community reintegration
- Maintain accurate records and documentation using our EMR system

Requirements:
- Bachelor's or Master's degree in Physical Therapy required; Doctorate preferred
- Licensed Physical Therapist in the state practicing in
- Completed state-regulated CEU requirements in NY and NJ
- Valid CPR license
- Experience/knowledge with EMR software preferred
- Excellent oral and written communication skills
- Strong clinical decision-making skills
