Job Title: Behavioral Health Specialist - Community Support Services
Job Summary:
Bridgeway Behavioral Health Services is seeking a highly motivated and compassionate Behavioral Health Specialist to join our Community Support Services team. As a key member of our team, you will provide resource development, skill development, and direct support to individuals with serious mental illness to help them integrate into their community of choice.
Key Responsibilities:
- Provide individualized support and guidance to clients to help them achieve their recovery goals
- Develop and implement personalized plans to address clients' needs and promote community integration
- Collaborate with multidisciplinary teams to ensure comprehensive care and support
- Conduct regular assessments and evaluations to monitor clients' progress and adjust plans as needed
- Maintain accurate and timely records of client interactions and progress
Requirements:
- Bachelor's degree in a related field (e.g., social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice)
- Minimum of 2 years of experience in a community-based behavioral healthcare setting
- Certification as a Psychiatric Rehabilitation Practitioner may be substituted for 1 year of experience
- Valid driver's license and reliable vehicle required
