WellBe Senior Medical is dedicated to providing a comprehensive, Physician-led, Advanced Practice Provider model of care tailored specifically for geriatric patients. Our focus is on serving the frail, multi-chronic elderly population who often face significant barriers to accessing healthcare.

We believe in bringing healthcare directly to our patients, ensuring they receive the necessary support without the challenges of traditional care settings.

Our services span the entire continuum of care, including chronic and urgent care in the home, hospital visits, skilled nursing facilities, assisted living, palliative care, and end-of-life support.

Our dedicated care teams collaborate closely with the patient's primary care physician to deliver high-quality geriatric medical care and social support, all within the comfort of the patient's home.


GENERAL ROLE DESCRIPTION
The Physician Assistant at WellBe is integral to embodying our mission and values while delivering care to our patients.

This role requires adherence to the regulations set forth by state and federal agencies, ensuring compliance in all aspects of care delivery.

As an advanced practice provider, you will be essential in managing the health of our patients within their homes, focusing on both chronic and acute conditions while promoting their ability to remain in their own living environments.

Your responsibilities will include managing a full patient panel, providing longitudinal primary care, urgent care, and acute medical interventions across a diverse patient demographic.

This position primarily involves delivering care in home-based settings, where you will be accountable for maintaining accurate and timely patient records.

As part of our ongoing care model, you will ensure that patients receive regular follow-ups, including initial assessments, urgent care visits, transitional care, and necessary follow-up appointments to address their health needs.

The ideal candidate will work collaboratively within our interdisciplinary team to provide timely, high-quality, and concierge-style care to every patient.

Conduct in-home patient visits to address care needs in your designated area.

Collaborate with other healthcare professionals within the Care Community to manage the health and well-being of patients in their home environments.

Engage patients in the WellBe program, supporting their care needs and maintaining their involvement.

Provide educational resources to facilitate informed decision-making among staff, patients, and families.

Perform initial patient evaluations, develop appropriate care plans, and ensure timely documentation.

Address urgent care requirements as they arise, including transitional care visits.

Work with the community team to conduct comprehensive patient assessments and develop care plans.


Order, interpret, and analyze diagnostic tests, determining the need for follow-up care and treatment options in accordance with state regulations.

Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents as permitted by state guidelines.

Maintain accurate patient records and verify their correctness at each appointment.

Deliver patient care and conduct regular reviews with supervising physicians as required by state regulations.

Ensure the highest quality of care while adhering to standards related to patient needs.

Focus on clinical care delivery to support documentation, coding, HEDIS measures, patient engagement, and managing healthcare costs.


Serve as a vital resource for patients, providing ongoing care information, counseling, and guidance for those with chronic conditions.


Utilize available resources to meet patient needs and refer to internal/external resources as necessary, including WellBe behavioral health protocols.

Collaborate with the interdisciplinary team and participate in regular team meetings.

Maintain active, unencumbered state licensure and national certification.

Provide palliative care and, when necessary, support end-of-life care for patients and their families.

Facilitate family meetings with the WellBe community team to address complex family dynamics and care goals.

Adhere to a professional code of conduct in patient care delivery, including completion of care needs and documentation standards.

Perform other tasks as needed to achieve team objectives.

Qualifications:

Educational and Experience Requirements:
A minimum of a Master's Degree in Physician Assistant Studies from an accredited program is required.

3-5 years of experience as a Physician Assistant is required; new graduates will not be considered.

2-3 years of prior healthcare experience in roles such as medical assistant, certified nursing assistant, EMT/paramedic, clinical laboratory scientist, or registered nurse is required.

Active, unencumbered Physician Assistant license in the state of New York is mandatory; state residency is required.

Current board certification with NCCPA is required.

BLS certification and an active NPI# as a PA are required.

CPR certification is required.

Active, unencumbered DEA licensure is required.

Experience in home care, primary care, hospice, palliative care, or geriatrics is strongly preferred; other complex healthcare experience is also beneficial.

Additional experience with telehealth care delivery is preferred.

A valid driver's license and proof of automobile insurance are required, as home visits to various residential settings are necessary.

Required Skills and Abilities:
Ability to conduct in-home visits and experience with telehealth platforms.

Proficiency in Microsoft Office, including Excel, Word, and G-suite.

Familiarity with office software, including Electronic Health Records and scheduling applications.

Demonstrated teamwork, integrity, ethics, and productivity.

Experience in diagnosing and treating ongoing, chronic, and urgent medical/psychological conditions in older adults.

Experience in panel management, primary care, geriatrics, internal medicine, and palliative care.

Strong communication skills, both verbal and written, with the ability to present effectively to various audiences.

Supervisory Responsibility:
This position will have supervisory responsibilities.

Travel Requirements:
Local travel may be required to deliver patient care.

Work Conditions:
Ability to lift up to 30 lbs.

Moving, lifting, or transferring patients may involve lifting up to 50 lbs and assisting with weights exceeding 100 lbs.

Ability to stand for extended periods.

Ability to drive to patient locations, including homes, hospitals, and skilled nursing facilities. Fine motor skills and visual acuity are necessary.
