Responsibilities:
Matrix Provider – Expectations:
Conduct comprehensive Adult/Geriatric Assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and thorough documentation in accordance with Matrix guidelines and protocols.
Collaborate effectively with physicians, case managers, social workers, family members, key caregivers, and any relevant ancillary medical personnel as necessary.
Engage with Primary Care Physicians (PCP) on patient education and provide follow-up care.
Deliver services across various settings, including Home Visits, Skilled Facility Visits, and Telehealth Consultations.
Nurse Practitioner or Physician Assistant - PRN/Part Time - Home Risk Assessments

PAY:
$115.00 per completed Assessment
Location:
Home, Skilled Nursing Facilities, Telehealth, and other community environments.
Hours:
Days, Evenings, Nights, Weekends, Flexible Scheduling
Benefits:
PRN/Part Time providers are non-benefited except for 401(k) and limited Voluntary Benefits.
Mileage Reimbursement plan.
Qualifications:
Essential Requirements:
Master's Degree required OR equivalent experience and successful completion of NP licensure.
Master's Degree required OR equivalent experience and successful completion of PA licensure.
Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
Board certified by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.
Possess a current BLS, ACLS, or CPR certification.
Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant. Ability to travel in-state or out-of-state.
Strong computer skills and familiarity with EMR software.
Excellent verbal and written communication skills with patients, clients, and colleagues.
A positive and outgoing personality, with the ability to work collaboratively in a team environment. Adaptability to frequent changes is essential.
Travel Requirements:
Ability to travel, possess a valid state driver’s license, and maintain adequate automobile insurance coverage.
Travel is necessary to meet patients in their residences.
