Job SummaryThe RN Case Manager Lead will supervise a team of RN Case Managers in the inpatient and emergency departments at all NCH campuses. The successful candidate will have strong leadership and team-building skills, possess excellent communication skills and problem-solving abilities.
Key Responsibilities- Support the Director of Case Management and Social Work Manager in implementing and enforcing NCH and departmental policies and procedures.
- Supervise, orient, and mentor RN Case Managers.
- Ensure comprehensive individualized discharge planning that meets the needs of patients and their families.
- Identify gaps in services and develop strategies to improve patient care and resource utilization.
- Participate and promote quality of care initiatives, length of stay, readmissions, and avoidable days.
- Minimum of Associate Degree in Nursing required; BSN preferred.
- A minimum of 2 years of supervisory/lead/manager level of experience in hospital Case Management is required.
- Licensed as a Registered Nurse in the state of Florida.
- Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
