Key Responsibilities- Provide accurate and comprehensive assessments, diagnoses, and treatment plans for patients with a range of medical conditions.
- Develop and implement individualized care plans, taking into account patient preferences, values, and goals.
- Collaborate with the clinical care team to ensure seamless care coordination and communication.
- Participate in clinical decision-making, including ordering diagnostic tests and prescribing medications as needed.
- Document patient care in a timely and accurate manner, using electronic health records.
- Master's degree in Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
- Minimum of 1 year of recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practice.
- Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work autonomously and make sound clinical decisions in a fast-paced environment.
