Nurse Practitioner

CVS Health is seeking a highly skilled Nurse Practitioner or Physician Assistant to join our Primary Care team. As a key member of our clinical care team, you will provide high-quality, patient-centered care to a panel of patients. Responsibilities- Provide evidenced-based care to patients, including accurate assessment, diagnosis, treatment, and management of health problems.
- Collaborate with the clinical care team to drive better outcomes for patients.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in clinical care team huddles and contribute to the development of care plans.
- Document patient care within an electronic health record in accordance with current policies and procedures.
- Work effectively with the clinical care team to ensure the delivery of high-quality, patient-centered care.
- Completion of a masters degree level Family Nurse Practitioner program or Master of Science in Physician Assistant Studies with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
- A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner or Physician Assistant in either a family medicine or primary care practice.
- Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
- Effective verbal, written, and electronic communication skills.
- Outstanding organizational skills and ability to multi-task.
