Nurse Practitioner

CVS Health is seeking a highly skilled Nurse Practitioner or Physician Assistant to join our Primary Care team. As a key member of our clinical care team, you will provide high-quality, patient-centered care to a panel of patients. This is a full-time position (30-35 hours/week) at our clinic located on SW Military. Bilingual is preferred.
Responsibilities- Provide evidenced-based care to patients, including accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
- Work collaboratively with a dedicated clinical care team to drive better outcomes for patients.
- Document all patient care within an electronic health record in accordance with current policies and procedures.
- Communicate effectively with members of the clinical care team, other health professionals, and health-related agencies.
- Participate in and lead clinical care team huddles, promoting mutual respect and trust within the team.
- Adapt quickly to new models of patient care for clinic efficiency, enhancing operational effectiveness while maintaining quality of patient care.
- Assist with hiring, development, and evaluation of Primary Care clinical care team members as needed.
- Completion of master's degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
- Or in select states, Master of Science in Physician Assistant Studies (or other health-related master's degree or equivalent experience together with Bachelor of Science in Physician Assistant Studies) with current National Board Certification and State of Employment license to practice in the role.
- A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner or Physician Assistant in either a family medicine or primary care practice preferred.
- Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
- Self-motivated, prioritizes and solves problems, takes initiative, and advocates for patients and practice.
- Effective verbal, written, and electronic communication skills.
- Outstanding organizational skills and ability to multi-task.
- Initiative, problem-solving ability, adaptability, and flexibility.
- Ability to work remotely in a clinical care team culture.
- Ability to work without direct supervision and practice autonomously.
- Proficient with information management and technology.
While performing the duties of the job, the Primary Care NP/PA is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responses. Specific vision abilities include the ability to view and read a computer screen and other electronic devices.
