About the Role

We are seeking a highly skilled Mental Health Therapist and Case Manager to join our team at Health Care for the Homeless. As a key member of our interdisciplinary care team, you will provide direct mental health therapy and case management services to adults experiencing homelessness with various disabling conditions and psychosocial stressors.

Key Responsibilities
  • Provide Behavioral Health Assessments and Diagnosis: Conduct thorough assessments and diagnoses to develop individualized treatment plans that address the unique needs of each client.
  • Deliver Individual and Group Therapy: Offer evidence-based therapy services to support clients in achieving their goals and improving their overall well-being.
  • Provide Intensive Case Management Services: Work closely with clients to develop and implement comprehensive case management plans that address their housing stability, health, and wellness needs.
  • Collaborate with Interdisciplinary Care Teams: Work effectively with other healthcare professionals to ensure seamless care coordination and delivery.
  • Develop and Implement Integrated Treatment Plans: Collaborate with clients to develop SMART goals and implement treatment plans that address their unique needs and circumstances.
  • Act as a Liaison between Clients and Landlords/Property Managers: Facilitate communication and coordination between clients and landlords/property managers to ensure smooth transitions and housing stability.
  • Ensure Relevant Authorizations are Completed: Complete necessary authorizations to facilitate billing for therapy services and ensure compliance with established guidelines.
  • Document Client Information in Electronic Health Records: Maintain accurate and up-to-date documentation of client information in electronic health records, adhering to established formats and timeframes.
  • Provide Representative Payee Services: Assist select clients with creating a monthly budget, ensuring monthly expenses are paid, distributing funds for individual use, and providing financial literacy education.
Requirements
  • Master's Degree from an Approved School of Social Work: Possess a Master's degree from an accredited school of social work.
  • Current License as an LMSW or LCSW-C: Hold a current license as an LMSW or LCSW-C.
  • Personal Vehicle and Valid Maryland Driver's License: Possess a personal vehicle and a valid Maryland driver's license.
Preferred Qualifications
  • One Year Experience Providing Therapy to Individuals with Substance Use and Mental Health Disorders: Possess one year of experience providing therapy services to individuals with substance use and mental health disorders.
  • One Year Experience Providing Case Management Services to Individuals with Substance Use and Mental Health Disorders: Possess one year of experience providing case management services to individuals with substance use and mental health disorders.
Skills and Knowledge
  • Knowledge of Assessment and Treatment Planning: Possess knowledge of assessment and treatment planning principles and practices.
  • Knowledge of Liberation-Focused Frameworks and Anti-Oppressive Methods: Demonstrate knowledge of liberation-focused frameworks and anti-oppressive methods, including their application in supporting and uplifting BIPOC and TGNC individuals and communities.
  • Willingness to Integrate Principles into Practice: Be willing to integrate principles such as Harm Reduction, Motivational Interviewing, and Housing First into practice.
  • Excellent Verbal and Written Communication Skills: Possess excellent verbal and written communication skills.
  • Strong Organizational and Time Management Skills: Demonstrate strong organizational and time management skills.
  • Able to Cope with Interruptions and Be a Team Player: Be able to cope with interruptions and work effectively as a team player.
  • Flexible Approach and Positive Attitude: Possess a flexible approach and a positive attitude towards change and new challenges.
  • Able to Work with Clients with Significant Mental Health Challenges: Be able to work effectively with clients who have significant mental health challenges, manage crisis situations, and assess for safety.
