Job Title: RN Case Manager

About the Role

Enhabit Home Health & Hospice is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a vital role in coordinating patient care, ensuring continuous assessment of each patient's and family's needs, and implementing the interdisciplinary care plan.


Responsibilities

Key responsibilities of the RN Case Manager include:

  • Providing coordination of care and ensuring delivery of care is compliant with federal and state regulations.
  • Performing services in accordance with the physician's orders and the established plan of care (POC).
  • Ensuring delivery of care is compliant with federal and state regulations.

Qualifications

To be successful as a RN Case Manager, you will need:

  • A graduate degree in nursing from an approved school of clinical education.
  • A valid state license to practice nursing.
  • At least one year of experience as a licensed professional.
  • Basic demonstrated technology skills, including operation of a mobile device.
