Job SummaryWe are seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will be responsible for coordinating care, assessing patient needs, and implementing the interdisciplinary care plan. You will work closely with our interdisciplinary team to ensure delivery of care is compliant with federal and state regulations.
Responsibilities- Coordinate care and ensure continuous assessment of each patient's and family's needs
- Implement the interdisciplinary care plan in accordance with physician's orders and established plan of care (POC)
- Ensure delivery of care is compliant with federal and state regulations
- Work closely with the interdisciplinary team to ensure seamless care coordination
- Graduate of an approved school of clinical education
- Licensed in the state where they currently practice
- At least one year experience as a licensed professional
- Basic demonstrated technology skills, including operation of a mobile device
- Experience as a licensed professional may be deferred with a review and approval from the regional and executive service line clinical leadership
- Medicare home health or hospice experience is preferred
- Valid state driver license
- Maintain automobile liability insurance as required by law
- Maintain dependable transportation in good working condition
- Ability to safely drive an automobile in all types of weather conditions
- CPR certification for the healthcare provider
