Medicaid Care Manager

Family and Childrens Association is seeking a dedicated and compassionate Medicaid Care Manager to join our team.
About the RoleThe Medicaid Care Manager will provide critical support, linkage, and coordination of services to children and youth ages 5 to 21 years old who are diagnosed with a serious emotional disturbance, chronic condition, or complex trauma.
Responsibilities- Provide case management services to ensure the best possible outcomes for children and families.
- Complete and submit all assessments, care plans, progress notes, and other written work in compliance with agency and regulatory standards.
- Conduct ongoing assessments and documentation of child and family strengths and needs, progress towards achieving goals, and efficacy of delivered services.
- Maintain contact and hold interdisciplinary team meetings with providers and families to maximize care.
- Participate in all mandatory trainings and any recommended by your supervisor.
- Keep both electronic and paper files up to date and in accordance with program, agency, county, state, and federal guidelines.
- Deliver and document all services using a strength-based and family-driven approach.
- Additional tasks as assigned to support the operations of the program.
- BA in Human Service-related field.
- Minimum of two years of related experience required.
- Experience with children is preferred.
- Excellent verbal and written communication skills.
- Intermediate computer skills.
- Valid and clean Driver's License.
