MelroseWakefield Healthcare is seeking a skilled Clinical Social Worker II to join our team. As a key member of our healthcare team, you will provide a range of clinical social work services to patients and families in inpatient, ambulatory, and community settings.

Key Responsibilities
  • Conduct psychosocial assessments of individuals and families to identify strengths, limitations, and deficits in their current situation and environment.
  • Develop treatment plans with appropriate interventions to address patient needs.
  • Provide crisis intervention services, including assessing risk for suicide, homicide, family violence, elder or child abuse.
  • Collaborate with healthcare teams to promote a smooth, coordinated plan of care.
  • Provide medical social services to patients' family members or caregivers as needed.
  • Identify and address psychosocial issues that may impede patient progress or recovery.
  • Advocate for patients and families with various systems, both internally and externally.
Requirements
  • Master's degree in Social Work.
  • Licensed Clinical Social Worker (LCSW).
  • One year of related experience.
