About the Role:

The Advanced Practice Provider will be responsible for the proficient management and coordination of patient care within their assigned population. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.

Key Responsibilities:
  • Coordinate and deliver exceptional and appropriate patient care.
  • Communicate effectively with nursing staff, collaborating physicians, and the Director of Nursing (DON) regarding patient care.
  • Conduct patient rounds, contribute to daily care decisions, write orders, and facilitate scheduling of therapeutic and diagnostic tests and procedures.
  • Record daily visit notes in an electronic medical record in a timely manner, with dictation services available.
  • Maintain accurate records related to professional services, including attending to billing reports, claims, and correspondence.
Requirements:
  • Excellent verbal and written communication skills.
  • Demonstrated ability to provide empathy and compassion.
  • Self-motivated, flexible, and a collaborative team player.

Education and Experience:

One year of relevant experience preferred. New graduates are still encouraged to apply. Nursing experience in Geriatrics, Internal Medicine, ICU, Emergency Medicine, Hospitalist, or General Practice preferred.
