Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Bristol Healthcare Center, a skilled nursing, assisted living, and memory care community in Waverly, OH.

Job Summary:

The Licensed Practical Nurse will provide high-quality patient care, work collaboratively with our healthcare team, and contribute to our mission to advance better living and care for seniors.

Responsibilities:
  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Conduct patient assessments, identify potential health risks, and develop strategies to mitigate them.
  • Participate in quality improvement initiatives and contribute to a culture of excellence.
Requirements:
  • Ohio LPN licensure in good standing.
  • Graduate of an accredited school of nursing or practical nursing.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
