Position Overview:
As a Registered Nurse, you will deliver direct nursing services in alignment with established protocols and guidelines within your designated specialty area.

The compensation range for this role is $65,000 - $80,000 annually.

Key Responsibilities:
1. **Patient Care Management**
- Engage in the coordination of patient care as part of a multidisciplinary team focused on high-risk populations.
- Provide health education as directed by healthcare providers or based on nursing assessments.
- Collaborate with the healthcare team to identify, assess, and prioritize the needs of patients and their families to formulate comprehensive care plans.
- Ensure compliance with all relevant regulations governing pharmacy operations.

2. **Triage and Follow-Up**
- Conduct telephone and walk-in triage for patients using established national standards.
- Perform medication reconciliation in collaboration with providers during pre-visit assessments and care coordination, including managing refills.
- Follow up with patients as instructed by healthcare providers.
- Conduct Birth Control Refill visits and provide necessary patient education.

3. **Team Collaboration**
- Refer to and utilize other extended care team members (such as Unit Clerks, Medical Assistants, Health Educators, and Referral Specialists) to enhance patient flow and provider efficiency.

Additional Functions:
1. **Quality Assurance**
- Participate in chart audits as per established protocols to ensure quality of care.
- Route incoming patient records and documentation appropriately.
- Communicate home health referrals and inform providers as necessary.

2. **Team Support and Clinic Operations**
- Engage in care team huddles and staff meetings as required.
- Participate in identifying and resolving issues related to job functions.
- Maintain effective communication with unit staff, other personnel, clinic management, and all departments, serving as a resource for other team members.
- Support decision-making processes, policies, and procedures.
- Engage in quality improvement initiatives, including policy and procedure development and updates.
- Identify opportunities for workflow enhancements and patient care improvements, collaborating to implement these changes.
- Adhere to HIPAA regulations.

Qualifications:
Education:
- RN, graduate of an accredited nursing program.

Experience:
- A minimum of two years of relevant experience is preferred.

Licensure/Certification:
- Current professional nursing license in the State of Texas.
- CPR Certification.

Knowledge, Skills, and Abilities:
- Bilingual in English and Spanish.
- Strong written and verbal communication skills, including active listening.
- Proficient in public relations and group presentations.
- Basic counseling skills and knowledge of health education, including human anatomy and physiology.
- Ability to establish and maintain professional relationships with patients, volunteers, and clinic staff.
- Capable of developing and utilizing resource and referral systems.
- Ability to work independently, demonstrating personal initiative.
