Job Summary
The Registered Nurse (RN) will provide care and services to a group of patients, visiting them in their homes, skilled nursing facilities, and assisted living facilities. This role is a contributing member of the interdisciplinary team, providing expertise in psych-social assessments, interventions, and community resources. The RN will work in either a Case Management, Admissions, or Resource role.
Responsibilities
- Provide High-Quality Patient Care: Deliver compassionate and individualized care to patients and their families, addressing their physical, emotional, and spiritual needs.
- Collaborate with Interdisciplinary Team: Work closely with nurses, social workers, chaplains, and volunteers to develop and implement patient care plans.
- Manage Patient Records: Maintain accurate and up-to-date patient records, utilizing our electronic medical records system.
- Communicate Effectively: Develop strong relationships with patients, families, and healthcare professionals through effective communication and empathy.
- Meet Regulatory Requirements: Ensure compliance with hospice rules and regulations, maintaining the highest standards of care and service.
Requirements
- Current RN Licensure: Hold a current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
- Strong Communication Skills: Possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Time Management and Organization: Demonstrate strong time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Electronic Medical Records System: Have experience using electronic medical records systems, with a willingness to learn and adapt to our system.
- Reliable Transportation: Possess a valid driver's license, reliable transportation, and auto insurance, with the ability to travel across the service area.
