Job Summary

We are seeking a highly skilled Clinical Social Worker to join our team at Access-Supports for Living. As a Clinical Social Worker, you will be responsible for providing clinical intervention, counseling, and support services to individuals in their homes and community settings.

Key Responsibilities
  • Provide crisis intervention and counseling services to individuals in their homes and community settings.
  • Develop and implement individualized service plans to support participants in achieving their goals and improving their quality of life.
  • Collaborate with case managers and other service providers to ensure participants receive comprehensive and coordinated care.
  • Provide training and guidance to case managers on evidence-based practices and recovery-oriented approaches.
  • Conduct assessments and evaluations to identify participants' needs and develop plans for intervention.
  • Provide customized individual and group training in specific life skills to address participants' needs.
  • Assist participants in accessing community resources and services, including entitlements and benefits.
  • Maintain accurate and up-to-date records and documentation of participants' progress and services provided.
  • Participate in case management and professional development activities, including trainings, webinars, and continuing education services.
Requirements
  • New York State Licensed Clinical Social Worker (LCSW, LMSW, LMHC) with a minimum of 2 years of experience working in a mental health/substance abuse field.
  • Ability to work with diverse populations and provide culturally sensitive services.
  • Strong communication and interpersonal skills, with the ability to work effectively with participants, case managers, and other service providers.
  • Ability to maintain a professional demeanor under pressure and operate with a keen sense of urgency and commitment to quality.
  • Driver's license and ability to travel to participants' homes and community settings.
Working Conditions

The Clinical Social Worker will work in a fast-paced, dynamic environment, providing services to individuals in their homes and community settings. The position requires the ability to sit or stand for long periods, move throughout the workday, and work in open spaces.
