You will develop a client-focused service plan collaboratively with primary care physicians and other decision-makers, ensuring seamless care transitions and optimal health outcomes.
Key Responsibilities- Care Coordination: Assess health care needs and develop a comprehensive service plan to ensure continuity of care.
- Health Functional Assessment: Conduct a face-to-face or in-home assessment to identify health care needs and develop a plan to address them.
- Service Plan Development: Collaborate with primary care physicians and other decision-makers to develop a client-focused service plan.
- Documentation and Communication: Maintain accurate and up-to-date documentation in the electronic health record and communicate effectively with care teams and community agencies.
- Resource Identification and Referral: Identify and refer members to community resources, including dental services, social services, and behavioral health services.
- Education: Master's degree in social work or a minimum of six years of experience in a directly related field.
- License: Licensed Clinical Social Worker (Hawaii) or Licensed Social Worker (Hawaii).
- Experience: One year of social work experience, with a focus on care coordination and case management.
