Job Summary:

The Qualified Mental Health Professional – Community Services (QMHP-CS) is a critical role within Youth Advocate Programs, Inc. (YAP), a leading organization dedicated to providing behavioral health services to youth and families.

Key Responsibilities:

  • Provide Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families.
  • Deliver case management and skills-based interventions to increase the youth's natural supports and strengths, fostering resilience and recovery.
  • Accurately document services, attend Wraparound Team meetings as needed, and communicate participant progress to supervisors, team members, and the referring authority.
  • Ensure all services are delivered in compliance with the Texas Medicaid Provider Procedures Manual, the Texas Resilience and Recovery Utilization Management Guidelines, YAP standards, and all state and federal regulations governing the delivery of service.

Requirements:

  • Bachelor's or Master's degree in a human services related field, as defined by Texas Administrative Code.
  • Experience in the provision of home and community-based services.
  • CPR/First Aid Certification.
  • Behavior Management (MANDT) Certification.
