Job Summary:We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at Samaritan Hospital. As a RN Case Manager, you will play a vital role in collaborating with the healthcare team to manage patient care and ensure optimal outcomes.
Responsibilities:- Collaborate with the healthcare team to develop and implement individualized care plans that address the unique needs of each patient.
- Provide education and support to patients and their families to promote health literacy and self-management.
- Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address any identified gaps in care.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions of care.
- Participate in quality improvement initiatives to enhance patient outcomes and promote a culture of excellence.
- Current and valid NYS RN license.
- Bachelor of Science in nursing, or willingness to pursue defined plan for matriculation in program demonstrated by ongoing coursework each semester.
- 2 years of experience in acute setting.
- Preferred requirements: 1+ year of experience in case management or utilization management, Case Management certification (CCM) or Certified Professional in Healthcare Management (CPHM), Discharge planning and community resource knowledge.
