About the Role

Key Responsibilities
  • Oversee the disease registry database to ensure accurate and up-to-date information.
  • Conduct pre-visit chart reviews to identify patient needs and develop effective care plans.
  • Coordinate care across the continuum to ensure seamless transitions and optimal patient outcomes.
  • Participate in quality improvement activities to drive continuous improvement and excellence in patient care.
Requirements
  • Graduate of an accredited school of practical nursing.
  • 2 years of ambulatory care experience, with a preference for 3 years of experience in ambulatory care and home health care.
  • Current licensed practical nurse (LPN) license in the state of practice.
Preferred Qualifications
  • Proficiency in using computers, software, and web-based applications.
  • Effective verbal and written communication skills, with the ability to present information clearly and professionally to varying levels of individuals.
  • Ability to be highly motivated, eager to learn new skills, and willing to obtain certification.
  • Organizational, decision-making, and delegating skills.
  • High level of positive interpersonal and public relation skills.
  • Leadership skills and ability to set priorities.
