Job Description

As a Patient Care Home Healthcare Coordinator, you will be responsible for completing admissions or visits for our Home Care team. This involves direct patient care under the supervision of the Director of Nursing or Nursing Supervisor, creating and/or following the plan of care, and providing and determining treatments for our patients.

Required Skills and Qualifications

To be successful in this role, you must possess a current RN license in the state of Minnesota, with at least two years' experience as an RN. A valid driver's license, reliable transportation, and proof of auto insurance are also required. One year of homecare or hospice experience is preferred, but not necessary.

Additional Responsibilities

You will also be responsible for completing CPR certification and traveling within our geographic service area. We encourage teamwork and collaboration, and as a member of our interdisciplinary team, you will have the opportunity to contribute to the development of individualized care plans.
