Key Responsibilities:- Manage Patient Care: Perform comprehensive nursing assessments, collaborate with the nursing team and other disciplines to develop effective plans of care, and establish realistic patient goals with interventions to minimize rehospitalizations.
- Lead Interdisciplinary Team: Delegate patient care responsibilities to staff, supervise and ensure that tasks and interventions are implemented, and communicate patient information with assigned staff and between shifts.
- Improve Patient Outcomes: Continuously evaluate patient outcomes, identify areas for improvement, and implement evidence-based practices to enhance patient care and satisfaction.
- Competitive Pay: $40.00 per hour
