Position: Senior Pediatric Care Coordinator

Work Location: Remote

Contract Duration: 6 Months

Compensation: $48/hr.

Weekly Hours: 40

Daily Schedule: 8:00am – 5:00pm

Work Days: Monday to Friday

Key Responsibilities:

  • Engaging with members, parents, guardians, and providers on a daily basis.
  • Documenting interactions with members, parents, guardians, and providers accurately.
  • Completing necessary referrals, assessments, care plans, and other documentation in accordance with established policies.
  • Coordinating with vendors to secure required Durable Medical Equipment (DME) and Home Health services for members.
  • Collaborating with facility discharge planners and social workers to facilitate effective discharge planning.
  • Providing educational support to parents and guardians regarding child development, disease management, and medication administration.
  • Participating in staff development meetings, team discussions, and discharge planning rounds.
  • Assisting with grievances and appeals as necessary.
  • Coordinating care with Medical Directors as required.
  • Working alongside Behavioral Health, Utilization Management, and other departments as needed.

Performance Expectations:

  • Achieve a productivity standard of 75 member interactions per week after completing orientation with an assigned caseload.
  • Manage a minimum caseload of 55 members.

Onboarding Process:

  • Initial onboarding with the Training Department.

Preferred Background:

  • Experience as a Pediatric RN or Pediatric Care Manager.

Required Qualifications:

Registered Nurse (RN) license in Ohio.

Desirable Skills:

  • Extensive nursing experience, particularly in pediatric care.
  • Experience in care management.
  • Proficient in computer documentation, including Microsoft Word.
  • Strong communication skills across diverse age groups and educational backgrounds.
