About the Role

We are seeking a highly skilled and experienced Care Navigator Supervisor to join our team at Marin Community Clinics. As a key member of our healthcare operations team, you will be responsible for overseeing the day-to-day activities of our care navigators, ensuring that patients receive exceptional care and support.

Key Responsibilities
  • Care Navigator Supervision: Provide guidance and oversight to our care navigators, ensuring that they are equipped to provide high-quality care to patients.
  • Workflow Management: Collaborate with our care navigator/BH operations manager to standardize workflows and create new ones as needed, ensuring that our care navigation processes are efficient and effective.
  • Community Resource Management: Stay up-to-date with community resources and ensure that our care navigators have access to the information they need to support patients.
  • Performance Monitoring: Monitor the daily work and performance of our care navigators, providing feedback and coaching to help them improve.
  • Staff Supervision: Supervise care navigator staff within your assigned work area, ensuring that they are meeting their performance expectations.
  • Communication: Communicate effectively with our care navigation team, ensuring that all team members are informed and aligned.
  • Incident Management: Handle site-specific care navigator-related incidents, ensuring that they are resolved promptly and effectively.
  • Training and Development: Train lead care navigators and ensure that they understand their role and responsibilities.
  • Staffing and Recruitment: Understand staffing requirements and create, execute, and support staff recruitment and retention strategies.
  • Meeting Coordination: Assist with the coordination of care navigator meetings and lead care navigator meetings.
  • Attendance and Tardiness: Monitor staff attendance and tardiness, enforcing expectations for communicating tardiness and absences.
Requirements
  • Education and Experience: 3 years of supervisory experience, or 3 years of related experience and/or training in a healthcare or social services field, or an equivalent combination of education and experience.
  • Medical Experience: At least 2 years of work experience in a medical clinic setting, preferably in a Federally Qualified Health Center.
  • Certifications: Medical Assistant Certificate from an accredited school, preferred. Current CPR certification, preferred.
  • Technical Skills: Electronic Health Records experience, preferably in Epic EHR.
  • Language Skills: Bilingual (Spanish/English) language skills, required.
  • Soft Skills: Ability to work in a fast-paced clinic environment, provide excellent customer service, multitask, and prioritize tasks effectively.
