Hospice RN Case Manager Position

As a Hospice RN Case Manager with Interim HealthCare Staffing, you will play a vital role in providing exceptional hospice care to patients and families. Your compassion, expertise, and leadership skills will make a significant difference in the lives of others.
Key Responsibilities:
- Maintain a specified case management load to ensure seamless care delivery.
- Conduct initial and ongoing assessments of patients' physical and psychosocial status to determine their needs.
- Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
- Coordinate care among IDG members and other healthcare providers to ensure continuous assessment and implementation of care plans.
- Supervise and direct services provided by LPNs, Hospice Aides, and Homemakers to ensure high-quality care.
Requirements:
- Graduate of an accredited school of nursing with a valid RN license in South Carolina.
- At least 12 months of accumulated experience as a registered nurse within the last five years or current clinical knowledge.
- One year of hospice care experience and one year of precepting or training experience preferred.
- CPR certification and a valid driver's license with auto liability insurance.
