Hospice RN Case Manager Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a Hospice RN Case Manager, you will be responsible for providing high-quality, patient-centered care to individuals and families in need of hospice services.

Key Responsibilities:

  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Collaborate with the Interdisciplinary Group (IDG) to ensure seamless care coordination and implementation of the IDG plan of care.
  • Provide education and support to patients and families to promote self-care and independence.
  • Manage and coordinate the delivery of hospice services, including medication management and symptom control.
  • Document patient progress and outcomes in accordance with regulatory requirements.

Requirements:

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.
  • One (1) year of hospice care experience.
  • CPR certification.
