Sutter Health is seeking an experienced Ambulatory Clinical Pharmacist III to join our team. As a key member of our healthcare team, you will be responsible for delivering high-quality patient-centered care through the provision of clinical services, medication information, education, and medication preparation and distribution.
Key Responsibilities- Deliver pharmaceutical care through the provision of patient-centered clinical services, medication information, education, and medication preparation and distribution.
- Exercise sound judgment and developed clinical skills to provide input to and implement the patient's plan of care based on diagnosis in a timely manner.
- Gain confidence and cooperation from patients, their families/support groups, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication.
- Adhere to all local, state, and federal regulations, codes, policies, and procedures to ensure privacy and safety while delivering optimal patient care.
- Doctorate: Graduation from an accredited school of Pharmacy.
- CERTIFICATION & LICENSURE: PHARMD-Licensed Pharmacist.
Days, Per Diem/Casual schedule, 8 hours per shift, Monday to Friday.
