Clinical Pharmacist Specialist

The Clinical Pharmacist Specialist is a key member of the Alameda Health System team, responsible for providing safe, effective, and cost-efficient medication therapy management to patients in the Ambulatory Care setting.
Key Responsibilities- Collaborate with patients, caregivers, and other healthcare professionals to develop and implement comprehensive drug therapy plans.
- Assess patient-specific medical problems, prioritize medical problems based on urgency and severity, and identify preventive and health maintenance issues.
- Evaluate patient-specific drug therapy, identifying potential interactions or adverse effects, and report and document drug errors, adverse drug reactions, allergies, and patient adherence issues.
- Design and implement a comprehensive drug therapy and patient education plan for patient-specific problems, justifying recommendations based on patient-specific pharmacologic, pharmacokinetic, pharmacodynamic, pharmacogenomic, pharmacoeconomic, ethical, legal, and evidence-based information.
- Under protocol, initiate, adjust, monitor, and recommend changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, physical assessments, review of medication profiles, and patient medical records.
- Document clinical interventions in the electronic health record in a timely and professional manner.
- Develop and maintain the drug formulary for AHS, providing education for physician and other healthcare providers regarding additions and revisions to the formulary, including non-formulary request review.
- Represent Clinical Pharmacy Services on various committees, staff meetings, and professional meetings as needed, serving as a drug information resource for other healthcare providers and team members.
- Coordinate continuing education activities for medical and nursing staff, orient new employees on pharmacy practices and procedures, and serve as a preceptor to pharmacy residents, students, and volunteers.
- Doctor of Pharmacy degree
- Completion of an American Society of Health-System Pharmacy accredited Postgraduate Year One (PGY1) Residency
- Postgraduate Year One (PGY1) Residency or Postgraduate Year Two (PGY2) Residency in Ambulatory Care is preferred
- Three to five years of recent ambulatory care pharmacy experience preferred
- Possession of a valid registered pharmacist license to practice in California
- Become Basic Life Support (BLS) certified
- Board certification (BCPS or BCACP) is preferred
- National Provider Identifier must be obtained upon hire
