Geriatric Healthcare Specialist

We are seeking an experienced and compassionate Registered Nurse to join our team as a Geriatric Healthcare Specialist at TLC Nursing Services.
About the RoleAs a key member of our healthcare team, you will be responsible for providing specialized nursing services to elderly patients, ensuring their physical and emotional well-being. You will collaborate with a multidisciplinary healthcare team to develop and implement care plans tailored to meet the unique needs of geriatric patients.
Main Responsibilities- Conduct comprehensive assessments of geriatric patients, including physical, cognitive, and psychosocial evaluations.
- Identify health concerns, chronic conditions, and potential risks associated with aging.
- Develop individualized care plans based on assessment findings, patient goals, and evidence-based practices.
- Collaborate with the healthcare team to coordinate care and ensure holistic treatment approaches.
- Administer medications and treatments as prescribed by physicians, monitor medication effectiveness, and educate patients and their families on proper medication usage and potential risks.
To be successful in this role, you must have:
- A valid nursing license and registration as a Registered Nurse (RN) in the relevant jurisdiction.
- Specialization or certification in geriatric nursing is preferred.
- Strong knowledge of geriatric health issues, common conditions, and evidence-based nursing interventions.
- Ability to assess, plan, implement, and evaluate care plans for geriatric patients.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
- Compassion, empathy, and patience in dealing with the unique challenges faced by geriatric patients.
- Proficiency in electronic medical records (EMR) systems and healthcare technology.
- Physical stamina and ability to perform tasks that involve standing, walking, lifting, and bending.
- Flexibility to adapt to changing work schedules, including evenings, weekends, and holidays if required.
