Hospice Aide Job Description

We are seeking a compassionate and dedicated Hospice Aide to join our team at Illinois Employer.

Key Responsibilities:
  • Follow the interdisciplinary plan of care, being alert to the patient's needs and concerns.
  • Provide personal care, including bathing, dressing, grooming, hair, nail, skin, and foot care, and oral hygiene.
  • Assist patients according to the plan of care, performing simple procedures as an extension of therapeutic services.
  • Observe and report changes in the patient's condition and needs to the Team Director or Registered Nurse.
  • May perform simple homemaking chores for the patient directly, meal preparation, and/or light housekeeping in the patient's immediate area.
  • Complete required documentation per policy and participate in continuing education programs.
  • Demonstrate a compassionate attitude toward patients and families consistent with the philosophy and mission of the organization.

Requirements:

  • Ability to work collaboratively as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality regarding patients and families.
