- Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines
- Current RN licensure in the state of Wisconsin
- Associate's degree in nursing or higher
- 1 year of experience in hospice or home health care, preferred
- Competitive hourly rate: $37.00 - $45.00
