Responsibilities:
Develop and implement individualized patient care plans in collaboration with the healthcare team.
Conduct thorough assessments of patients' physical, emotional, and spiritual needs, identifying areas for improvement and developing strategies to address them.
Provide education and support to patients and their families on disease management, symptom control, and end-of-life care options.
Collaborate with the healthcare team to coordinate care, including ordering and administering medications, performing procedures, and documenting patient information.
Participate in interdisciplinary team meetings to discuss patient progress, address concerns, and develop care plans.
Requirements:
Graduate of an accredited school of nursing with a current RN license in the state of practice.
Experience in hospice or palliative care is preferred, but not required.
Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities simultaneously.
