Job Description: RN Stroke Program Coordinator

Adventist Health is seeking a skilled RN to join our team as a RN Stroke Program Coordinator. This is a per diem opportunity located in Saint Helena, CA.

About the Role:

The RN Stroke Program Coordinator will work closely with the multidisciplinary team to provide coordinated care for stroke patients. The ideal candidate will have excellent communication skills and be able to navigate complex healthcare systems to ensure timely and effective care.

Key Responsibilities:

  • Develop and implement care plans for stroke patients, ensuring timely and effective interventions
  • Collaborate with the team to identify and address barriers to care, including transportation and financial concerns
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions of care
  • Maintain accurate records and reports, including volumes and outcomes

Requirements:

• Registered Nurse (RN) licensure in the state of practice

• 2+ years of experience in a hospital setting, preferably in stroke care

Estimated Salary: $85,000 - $130,000 per year, depending on experience
