Job Summary
As a compassionate and skilled Nurse Navigator, you will play a vital role in guiding patients through the clinical care system, ensuring timely treatment and recovery for stroke patients. This dynamic position requires strong communication and interpersonal skills, as well as a deep understanding of the healthcare system.

About the Role
You will serve as a patient advocate, establishing contact with new patients and/or family members, or caregivers. You will understand clinical care options and direct patients to healthcare services within the organization, at outside facilities, and within the community. Your expertise will help identify population and patient-specific barriers to care and connect patients with resources, healthcare, and support services in their communities.

Key Responsibilities
- Assist patients and families in identifying and accessing appropriate institutional and community resources
- Implement care interventions and follow-up to ensure timely care, preventing delays and access to services
- Interact with hospital departments to fulfill patient needs
- Collaborate with the team to provide patient care in a safe, healing, humane, and caring environment
- Provide learning opportunities for patients, families, and team members
- Document coordinator and patient interactions in electronic or hardcopy chart
- Create reports from patient data such as volumes and outcomes

Requirements
- Bachelor's Degree in Nursing (BSN): Preferred
- Relevant clinical experience: Preferred
- Nursing experience with stroke-specific emphasis: Preferred
- Licenses/Certifications: Registered Nurse (RN) licensure in the state of practice: Required
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Required
