Job Summary

We are seeking a dedicated and compassionate Direct Service Provider to join our team at Horizon CFS LLC. As a Direct Service Provider, you will play a vital role in supporting individuals with unique needs, promoting their independence, and enhancing their quality of life.

Key Responsibilities
  • Develop and maintain a deep understanding of each individual's needs, goals, and preferences.
  • Provide personalized support and assistance to help individuals achieve their goals and maintain their independence.
  • Collaborate with families, caregivers, and other professionals to ensure a comprehensive and coordinated approach to care.
  • Document progress, maintain accurate records, and submit reports in a timely manner.
  • Participate in regular team meetings and training sessions to stay up-to-date on best practices and protocols.
  • Adhere to all documentation requirements and submit progress notes, service plans, and expense logs in a timely manner.
Requirements
  • Associate's degree or higher in a related field.
  • At least one year of experience in a related human services role.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work varied shifts, including evenings and weekends.
Preferred Qualifications
  • Bilingual or multilingual skills.
  • Experience working with adolescents or individuals with special needs.
  • Knowledge of community resources and services.
  • Strong documentation and record-keeping skills.
