Care Transition Specialist

We are seeking a skilled Registered Nurse (RN), Licensed Practical Nurse (LPN), Physical Therapist (PT), Physical Therapy Assistant (PTA), or Licensed Master Social Worker (LMSW) to fill the role of Care Transition Coordinator.
About the RoleThis position involves coordinating patient care transitions to ensure seamless continuity of services. The ideal candidate will possess excellent communication and organizational skills, with the ability to work effectively in a fast-paced environment.
Key Responsibilities- Coordinate patient care transitions to ensure timely and effective delivery of services
- Collaborate with healthcare teams to develop and implement individualized care plans
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
- Monitor and evaluate patient outcomes to identify areas for improvement
To be successful in this role, you will need:
- A degree in Nursing (RN, LPN), Physical Therapy (PT, PTA), or Social Work (LMSW)
- Excellent communication and organizational skills
- Ability to work effectively in a fast-paced environment
- Strong analytical and problem-solving skills
