Job Summary:
In this role, you will provide developmental support services to adults with disabilities, promoting their choice, independence, and community inclusion. You will work collaboratively with our multidisciplinary team to develop and implement personalized support plans, ensuring the well-being and growth of our clients.
Key Responsibilities:
- Provide developmental support services to promote client choice and independence
- Collaborate with healthcare professionals, families, and other stakeholders to develop and implement personalized support plans
- Maintain accurate records and communicate effectively with colleagues and supervisors
Qualifications:
- Associate's degree in a related field or equivalent experience
- Valid driver's license and reliable transportation
- Knowledge of disability support services and relevant laws and regulations
Salary Range: $40,000 - $50,000 per year (dependent on experience)
