Job Overview:

This is a part-time opportunity as a Direct Service Provider. The successful candidate will provide essential services to members based on their personalized service plans.

Key Responsibilities:

  • Cultivate a deep understanding of each member's unique needs and circumstances.
  • Provide educational resources and advocacy support tailored to the individual's requirements.
  • Enhance the member's integration into their caregiver/family unit and support caregivers/families in providing ongoing care at home and in community settings.
  • Promote self-sufficiency among caregivers/families by empowering them to access resources independently.
  • Address any concerns related to supporting the child or youth, respecting cultural and individual contexts.
  • Offer comprehensive instruction to deepen understanding of the condition or illness.
  • Engage with families/caregivers and children/youth to promote a holistic approach to accessing community services and activities.
  • Regularly update service plans, obtaining medical consenter signatures and ensuring plans reflect evolving needs.
  • Acquire necessary consents and documents for newly enrolled members.
  • Attend monthly meetings, ensuring alignment with best practices and protocols.
  • Report incidents promptly to the supervisor.
  • Collaborate openly with other service providers (e.g., Care Manager) to track progress, address potential barriers, and streamline referrals.
  • Submit documentation requirements (progress notes, service plans, expense logs, and service provider logs) in a timely manner.

Work Environment:

Services can be provided both in-home and in-community settings, emphasizing a holistic, integrated approach.

Requirements:

  • High School Diploma or Equivalent Required.
  • Associates Degree preferred.
  • Bachelor's Degree preferred.
  • Bilingual Spanish Required.

Estimated Salary: $25/hour - $40/hour, based on experience and location.
