Senior Affordable Housing Case Manager

Civic Works is seeking a highly skilled and compassionate Service Coordinator to join our team. As a Service Coordinator, you will play a vital role in enhancing the quality of life for members in a senior affordable housing building.
Key Responsibilities- Collaboration and Communication: Work closely with members, the Co-op council, and other committees to ensure seamless service delivery.
- Assessment and Referral: Conduct thorough psychosocial assessments for residents at-risk or frail, and coordinate referrals to relevant services.
- Case Management: Provide general case management and referral services to support members' needs.
- Documentation and Reporting: Maintain accurate and up-to-date files on members, programs, and activities through an online case management program.
- Community Partnerships: Establish relationships with local service providers, monitor their quality and effectiveness, and ensure they meet established criteria.
- Support Network Development: Assist members in creating and maintaining informal support networks.
- Training and Evaluation: Participate in regular training, research, and evaluation of outcomes to continuously improve services.
- Liaison and Program Development: Act as a liaison between community agencies, service providers, and resident members, and plan and implement dynamic educational and group programs.
- Education and Experience: Associate degree or higher in social work, psychology, gerontology, counseling, or related specialty, with 1+ years of experience working with older adults in a community-based setting, or equivalent combination of education and experience.
- Knowledge and Skills: Must have knowledge of senior community and area resources, demonstrated ability to work independently, good oral, written, and computer skills, and proficiency in Microsoft Office products.
- Personal Qualities: Highly organized, detail-oriented, and able to manage multiple projects simultaneously, exceptional personal responsibility, excellent oral and written communication skills, and ability to recognize problems and develop innovative solutions.
