Job Summary

The YMCA of Central Maryland is seeking a highly motivated and experienced Case Manager to join our Transitional Housing Program team. As a Case Manager, you will be responsible for providing comprehensive case management services to homeless families, including assessment, goal setting, and service planning.

Key Responsibilities
  • Case Management: Work with a caseload of 12 families to provide individualized case management services, including assessment, goal setting, and service planning.
  • Service Coordination: Coordinate with community resources to provide wrap-around services, including housing, employment, education, and healthcare.
  • Documentation: Maintain accurate and up-to-date documentation, including HMIS requirements, case files, and client statistics.
  • Program Management: Manage the transitional housing program according to grant/contract specifications, including ensuring compliance with all relevant regulations and requirements.
  • Collaboration: Work closely with the Program Director and other staff members to develop and implement individual service plans, conduct assessments, and ensure enforcement of program housing rules and regulations.
  • Training and Development: Participate in staff meetings and agency training, and assist in the development and implementation of Continuous Quality Assurance activities.
  • Communication: Communicate the YMCA's mission and objectives to the community, and assist in facilitating groups and presentations.
Requirements
  • Education: Bachelor's degree in Social Work, Psychology, or a related field.
  • Experience: 2+ years of experience in case management, social services, or a related field.
  • Skills: Strong communication and interpersonal skills, ability to work with diverse populations, and proficiency in Microsoft Office.
