Qualifications for Community Resource Advocate:

While specific qualifications are essential, a combination of experience, education, and training that provides the necessary skills and knowledge is acceptable. Candidates must be able to perform physical tasks such as sitting, standing, and walking for extended periods. Proficiency in reading and understanding English is required, along with the ability to maintain confidentiality and communicate effectively across various levels of personnel.

Experience: Prior experience in a medical office environment is mandatory.

Bilingual Skills: Proficiency in English and Spanish is preferred but not mandatory.

Essential Skills: Familiarity with medical office software and comfort with daily computer usage is crucial. Candidates should possess excellent verbal communication and organizational skills, along with the ability to problem-solve and think critically while ensuring patient confidentiality.

Preferred Skills: Basic knowledge of medical terminology and proficiency in software used within the organization, including Epic, ACA software, SHIIP software, and TPC.

Educational Requirements: A High School Diploma or GED is required; a Bachelor's Degree is preferred.

Certifications: SHIIP Counselor, SOAR Counselor, ACA Navigator, and completion of TPC training are required.

Key Responsibilities:

  1. Deliver exceptional customer service to all stakeholders, including community members, patients, and agency staff.
  2. Utilize motivational interviewing techniques to assess individual needs and available resources.
  3. Provide education on program requirements and facilitate enrollment or referrals based on individual circumstances.
  4. Adhere to guidelines and policies related to MAP, Medicare, disability, ACA insurance, FQHC, and community agency programs.
  5. Engage in quality improvement initiatives to enhance processes and reduce errors.
  6. Collaborate with staff to ensure efficient workflow.
  7. Utilize data systems for effective communication and referral processing.
  8. Comply with all reporting, training, and certification obligations of SOAR, SHIIP, ACA, FQHC, and other community programs.
  9. Recruit volunteers to expand SHIIP and SOAR service capacity.
  10. Document activities in The Connector, TPC, and other required platforms.
  11. Foster positive relationships with community members, healthcare providers, and partners.
  12. Engage in community outreach to build and maintain relationships with agencies and members.
  13. Physical demands may include setting up equipment for off-site events.
  14. Perform additional duties as assigned.
